Policies
Payment Types
Payment can be made by cash, check or credit card at our offices at the time of your service. Checks should be made to “Boulder Therapeutics” at our Boulder office. Checks should be made to “Impact Sports Performance” in Superior.
Cancellation Policy
If you need to cancel your appointment, please cancel online or notify us by phone 24-hours in advance by calling the specific office where you are booked. Cancellations with less than 24-hr advanced notice will be charged 100% for the missed session (excludes emergencies).
Refund Policy
All pre-paid sessions gift cards & 5-packs must be used within 1 calendar year of purchase. Pre-paid sessions, gift cards and packages not used within 1 year can be refunded by written request, less any applied discounts & a $25 processing fee. Gift cards & 5-packs purchased can only be used at the offices where they are purchased, they cannot be used at our other locations…sorry, it’s an administrative thing. And Outcalls that are prepaid in full can receive 50% refund if cancelled with 72 hrs notice and no refund after 72 hours.
