Massage Therapy for Anthem BCBS Insurance

Our contract with Anthem Blue Cross and Blue Shield ended on 7/31/17 because they dramatically cut our reimbursement. Beginning, 8/1/17, we can treat all clients as as a privately paying but cannot bill your insurance.

We accept some Anthem BCBS insurance plans if your plan offers you the 20 combined visits of massage, acpuncture & chiropractic. Our talented massage therapists are trained to work with most injuries and medical conditions. We are only an approved provider for massage therapy treatments. (Fill out paperwork)

Get Preapproved by us!

First, call the number on the back of your card to confirm your massage therapy benefit. Ask your Anthem representative, “Is massage therapy a covered benefit if provided by a Massage Therapist?” If yes, then ask them to “check the ‘Exclusions’ list to verify that Massage Therapy is NOT excluded.” Then, please fill out the form below. Unfortunately, we do not accept other medical insurance plans at this time. **Preapproval is needed to verify your insurance and copay. Please note that Anthem can change, update or deny any policy or claim for any reason, even after being preapproved by us, which could cause you to responsible for payment of any treatment.

We do require your credit card information on the “Financial Agreement” form. We DO NOT accept Anthem insurance at our Superior location. Thank you!
